NYS Supervised Driving Form Question?

I living in NYS and I lost the yellow sheet that says you must have at least 50 hours of supervised driving and have it signed by a parent/guardian. I lost this sheet so do I have to go back the the DMV or can I just print it from the internet and if so where can I find it?
